Bully Incident Reporting Form

This form, when filled out, goes directly to the B.A.R. program administrator and will be forwarded to the appropriate School Principal or Bully Prevention Specialist / Committee. Though the form is anonymous, the more detail given will help the respective organizations resolve the situation and curb the tide of bullying in our schools.

Definitions

Bullying

A person is bullied when he or she is exposed, repeatedly and over time, to negative actions on the part of one or more other persons, and he or she has difficulty defending himself or herself.

Physical Bullying

Any bullying where hitting, kicking, pushing, grabbing, or other form of unwanted physical contact is present. These may include instances of theft.

Mental / Psychological Bullying

Any bullying where name calling, threats, excessive or cruel “practical joking”, teasing, etc is present. These may also include instances of theft.

Social Bullying

Any bullying where forcible exclusion, “ganging up”, rumors, lies, ostracization by a or from a social group, circle, or clique, etc is present

Cyberbullying

Any form of bullying which happens online, through social media, over email, instant messaging, chat rooms or text messaging. These may also include instances of account or hardrive “hacking”.

Falsely accusing any person with the intent to do harm or damage to them, their reputation, or their family and loved ones can lead to prosecution under the law and result in jail time, fines, or both. Such actions are a form of bullying and will be reported to the to the appropriate authorities.
